 What to add in the design to have a check box to enable an account or disable it.

I am wondering the same, did you find a solution yet? I did try the UserAccountControl but with not so good results.

Actually it works, i used the UserAccountControl field, the value to disable an account is 514 as the value for a normal user is 512. It just took a while to update our AD.

But your question was how to add a check box, simply change the type from text to check, this can be done via the display type drop down list.

 We use the UserAccountControl field to set the value 514 to disable an account.  i would like to add this UserAccountControl field in the Default SearchForm, but then, when I enter 514 in UserAccountControl field in the search form, nothing is found ! If I had UserAccountControl field in the search result, nothing is displayed !
